Treatment of COPD with mild/moderate PH with ET1/PDE5 inhibitors is not beneficial and may be harmful. There is evidence that sildenafil in patients with COPD and severe PH with/without right heart failure is beneficial to improve hemodynamics, exercise tolerance and QoL. Further large randomized studies are required to conclusively establish the role of PH-directed therapy in COPD.
